OCO Webinar: Advance Care Planning for Caregivers

In this webinar, we will cover two main topics. The first is a discussion of the fundamental aspects of advance care planning (ACP) as it applies to you and to the people you care for. We will discuss what ACP is, why it is important and how to begin advance care planning discussions. We will also discuss the role of a substitute decision-maker (SDM). The second topic will include a discussion of important concepts in caring for people with advanced illness, such as “what does resuscitation status really mean?” and “when should I have palliative care involved?” and “what is the difference between care in a hospital versus a palliative care unit or hospice?”

This webinar is presented by Dr. Leah Steinberg, a palliative care physician at Mount Sinai Hospital. She currently teaches palliative care to all level of learners and is involved in research in creating models for integrating palliative care into chronic disease management. Dr. Steinberg is also a member of the Advance Care Planning, Goals of Care, and Health Care Consent Clinician Advisory Team at Hospice Palliative Care Ontario. Dr. Jeff Myers will join Dr. Steinberg in this presentation.
