Most of us, towards the end of our life, need someone else to speak for us and make treatment decisions for us.  Who is that person for you- as defined by law?  How does that person know what we would say or choose?  What if others in the family disagree?  Who is capable of making their own medical decisions and who is not, how is that determined, and who does it?

When we finally face an illness that could take our life, how do we know what treatment options are right for us?

Advance Care Planning is about figuring out who our Substitute Decision Maker would be, and about how to have conversations with that person so that they know how to make decisions that we would make for ourselves if we were able.  It is also about how to figure out what treatment we would want when faced with a serious and life-threatening illness.
