This role represents an exciting opportunity to be part of a new program that aims to have a major impact on the caregiver experience in Ontario. You will be helping to enhance existing social prescribing principles to be more inclusive of the caregiver experience. The ideal candidate can work alone with minimal supervision, as well as on a team, and be able to carry out both program management and direct service delivery duties within the community.

About the role:
18-month contract position full-time, 37 hours per week (Monday to Friday). Ability for occasional work after hours or on weekends to accommodate pre-arranged meetings or events is required. Reporting to the Manager, Caregiver Navigation and Workplace Programs, the Caregiver Link worker will establish and maintain community-based social prescribing programs focused on caregivers within primary care and other community health settings. You will be helping to create and implement new models of social prescribing for caregivers.
Responsibilities:
Provider Engagement
- Identify and develop key partnerships with primary care teams or community health services
- Provide live in-person information sessions or lead in-person and virtual meetings with community partners
- Connect with health and community service providers and support them in-person as an extension of their team
Project Management
- Identify potential sites and build a successful new program within pilot sites
- Develop strategies, processes, and practices alongside a team for service delivery and referrals
- Independently manage project deadlines, workflow, and daily service delivery requirements
- Collect required data points and develop reports
- Provide input to strategic planning for the program and organization as a whole
Direct to Caregiver Support
- Manage incoming and outgoing referrals and communications in a timely manner
- Provide direct support to caregivers in-person, by phone, and email, listening to their needs and co-developing intervention options
- Provide resources or program options for their care recipient as needed
- Assist caregivers in navigating OCO programs as well as support within their community
- Complete service readiness screening and program registrations as required
Qualifications:
- Live in a community within northern, western, or eastern Ontario
- Bachelor’s degree in social sciences or similar discipline
- Registered Social Worker in good standing with the Ontario College of Social Workers and Social Service Workers is an asset
- Experience within primary care, community health settings, or clinical environments is an asset
- Highly skilled in supporting community members with complex needs
- Excel at facilitation of workshops, focus groups, network meetings and outreach
- Project or program management experience, managing deadlines and reporting requirements
- Ability to develop and maintain partnerships with external groups and providers
- Assessment capabilities to determine best service match
- Ability to navigate community and publicly funded supports and services
- Strong conflict resolution skills, with the ability to manage challenging situations
- Comfortable in working directly with vulnerable community members, facing hardship
- Experience with following privacy and consent guidelines
- Self-starter, solution focused with demonstrated problem-solving skills
- Strong technical ability and proficiency in the use of computers and various software applications, in particular Microsoft Office, Dynamics 365, Survey Monkey, OCEAN, Zoom
- Ability to speak and write in French is an asset
- Ability to travel within Ontario, Valid G licence and access to an insured and reliable vehicle
